Account Disclosure Form
The Account Disclosure Form allows cardholders to authorized third parties, as specified by the cardholder, to have access to personal account information, otherwise protected under FERPA or University privacy policies. Cardholders mailing this form must include a photo copy of their SCSU Hoot Loot ID Card and another form photo identification.
Hoot Loot Account Refund Form
Cardholders who have graduated, withdrawn or otherwise separated from the University may request a refund of their remaining Hoot Loot funds. Refunds will be issued according to the Terms of Agreement.
Hoot Loot Transfer Form
Cardholders may transfer Hoot Loot funds from their account to another cardholder's Hoot Loot account using this form.
Parent Loan Authorization Form
Please note that the University Card Office "Parent Loan Authorization Form" has been replaced by the Office of Financial Aid & Scholarships Federal Direct PLUS Loan Excess Fund Authorization Form. Click here to learn more about this form.
Vending Machine Refund Form
Cardholders who have lost money (cash or Hoot Loot) in a beverage, copier, snack or laundry machine must complete this form and return it to the University Card Office for reimbursement. Lost funds can only be credited to a Hoot Loot account. For cash refunds you must contact the individual vendor responsible for the machine (i.e. Coca Cola Enterprises for beverage machines or Berkshire Foods for snack machines).
